In order to be a member of FC Long Beach, every parent must fill out and sign the attached form annually. A copy of an official birth certificate must also be provided before we can register your child to a team.  Please download the CYSA player registration form, have it signed by parent and return to your coach and or team administrator with a copy of birth certificate.
